If you wish to apply for your student(s) to participate in the Distance Learning Independent Study Program for next school year, please read the information below and submit this form to apply. Please submit ONE FORM PER STUDENT. The submission of this form does not mean your student(s) are automatically enrolled. The District Administrator for the DLA Independent Study Program will contact you and schedule a meeting to fully enroll your student in the coming weeks.
